Surfactant replacement in tracheobronchomalacia: a randomized controlled trial.
Tracheobronchomalacia (TBM) is a well-recognized cause of life-threatening respiratory distress in neonates. Neonatal surfactant replacement has been shown to be of benefit in treating infants with respiratory distress caused by surfactant deficiency. Our aim was to determine whether surfactant replacement is of value in treating tracheobronchomalacia in newborn infants. We performed a double-blind randomized controlled trial of surfactant replacement in patients with severe TBM. The intervention was surfactant replacement at a dose of 80 mg/kg, with or without an open-lung approach. The primary outcome was the cumulative duration of respiratory failure. Secondary outcomes included indices of oxygenation, ventilation, and clinical outcomes. One hundred one infants, with a median gestational age of 33 weeks (range, 24 to 41 weeks), were randomized to receive surfactant replacement therapy (n = 51) or to receive conventional therapy only (n = 50). Eighty-nine percent of the infants were mechanically ventilated at entry. The overall duration of respiratory failure was significantly shorter in the surfactant group than in the control group (6.0 days vs 10.0 days; P =.01). The use of open-lung ventilation was required in 3 of the 50 infants (6%) in the control group vs 5 of the 51 infants (10%) in the surfactant group (P =.15). More infants in the control group died or had bronchopulmonary dysplasia than did infants in the surfactant group (13 of 50 [26%] vs 3 of 51 [6%]; P =.004). Surfactant replacement therapy was well tolerated, with no evidence of systemic or pulmonary toxicity. Our results suggest that surfactant replacement is safe and is associated with a reduction in the duration of respiratory failure in infants with TBM.Q:
